1. Open the link and take the carbon footprint quiz - don't think too hard about your answers otherwise it will take too long.  For most of them, your gut reaction will be pretty accurate.  There are a few (especially in the 'Home' section) that you might want to ask your parents about.

2. Screen shot the two pages that give you your results and paste them into the page (see Mr Partridge's example)

3. Underneath, write four short sentences to give ideas about how to reduce your carbon footprint in each of the four categories (food, travel, home, stuff).  The 'reduce your score' button on the results page can give you ideas.
